Personal Care Services may include:

1. Basic personal care and grooming

• Washing/bathing
• Hair care, including shampooing
• Care of nails
• Dental and oral hygiene
• Skin and foot care
• Shaving
• Application of make-up
• Dressing

2. Transfer assistance
Weight-bearing and non-weight bearing assistance, such as steadying the individual

3. Mobility assistance
Accompanying the individual as he or she moves from one location to another, opening doors, handing the individual his or her cane or walker, pushing a wheelchair, or another type of assistance

4. Assistance with toileting and hygiene measures

• toileting
• use of bed pan/urinal
• incontinence briefs
• emptying catheter 

5. Assistance with personal appliances

• Cleaning dentures, hearing aids, glasses
• Assistance with the application of some types of braces, splints, slings, and prostheses as specified in the care plan

6. Assistance with nutrition, hydration, and meal preparation

• Preparation of the meal
• Arranging food, including cutting up or mashing food
• Filling the individual’s fork or spoon
• Encouraging the individual to eat or drink
• Feeding the individual as specified in the care plan

7. Assistance with self-administration of medication as defined in Hem-E 801.2(d)

• Includes oral and topical medications
• Reminding the individual regarding the timing and dosage of the medication, and to take his or her medication as written on the medication container
• Placing the medication within reach of the individual
• Assisting the individual with opening the container
• Assisting the individual by steadying shaky hands
• Observing the individual take the medication and recording the same in the individual’s record

8. Essential household services

• Meal planning and preparation
• Assistance with paying bills
• Shopping for groceries, clothing or other items with funds provided by the individual
• Doing laundry
• Performing light housekeeping tasks, including washing dishes, dusting, vacuuming, sweeping, wet-mopping floors, cleaning kitchen and bathroom fixtures, and emptying wastebaskets
• Doing errands

9. Transportation 

• Medical Appointments
• Pharmacy Trips 
• Banking 
• Grocieries
