LSS SmartCare: A Consumer-Directed Personal Care Service Program
LSS SmartCare --
- Is an alternative to nursing home care
- Allows an individual to stay at home and direct his/her own personal care services, giving choice and control over personal care decisions
- Offers an alternative to traditional agency-directed home health agencies that determine who will come to the home to provide services and dictates the schedule of service
- Allows an individual to appoint a representative -- a spouse, family member, or other individual who is NOT the personal care service worker -- to make and manage personal care decisions
- Allows an individual to select and hire a personal care service worker of his/her choice, including a family member or friend (NOT the spouse, representative, parent, legal guardian or POA)
- Puts the scheduling of care in the hands of the individual served, according to his/her needs and preferences
- Allows the individual served to decide what services the personal care service worker will perform – the individual establishes the job tasks according to his/her wants and needs. For examples of what personal care services include, please click here.
To become an LSS SmartCare client -- You must want and be able to manage and direct your own care. If you wish, you may appoint a representative to manage and direct matters of personal care for you. Services are paid for through private pay or the HCBC-ECI (Home and Community-Based Care for the Elderly and Chronically Ill Medicaid).
