What does the LSS SmartCare In-Home Care Manager do?
Care managers are health and human service professionals who help seniors meet basic care needs as well as help to navigate the complex world of health care. The care manager performs a comprensive needs assessment and develops a personalized care plan. As an advocate for the client, a care manager works to ensure that the client's wishes are fulfilled while also promoting an optimal level of function and safety. The care manager takes into consideration the client's medical, financial, residential, cognitive, and other issues in creating the personalized care plan.
