LSS SmartCare In-Home Services is a new and innovative alternative for older adults and persons with disabilities or chronic illness. We help these individuals to remain in their homes and communities and to lead independent and satisfying lives by enabling them to select their own caregivers and manage their own care. LSS SmartCare In-Home Services offers two statewide programs:
• LSS SmartCare In-Home Consumer Directed Care is a program that gives people choice and control over decisions about their personal care needs.
The consumer, with assistance from LSS SmartCare In-Home staff and a representative if desired, hires, and trains staff on their personal care needs. The consumer supervises the employee as well as determines a schedule that is mutually agreeable. This service is provided through Medicaid or at a very competitive private pay rate.

Do you worry about safety at home?
Is your family worried?
Are you feeling overwhelmed by household chores, managing finances, and getting to appointments?
If you answered yes to any of these questions LSS SmartCare Care In-Home Management Services may be able to help you. This program is designed for persons who may need an array of services in order to stay at home or may require assistance to sort through the many support services that are currently being offered in the community. Some of the services provided are: access to transportation, making appointments, home repair and maintenance services, as well as assisting and or making referrals for legal and financial services.
